If you experience a qualified life status event* that affects your benefits, you must request any desired changes and provide supporting documentation within 31 days of the qualified life status event. You can do this online through COMPASS (log in when outside the Huber Network or use single sign-on access when inside the Huber Network if you have already registered). You may also call the J.M.   • If you’re changing your name, get a new Social Security card.
  • Provide your local HR with your updated last name (if applicable) and marital status and your proof documentation. (As a reminder, never send a document that contains your Social Security number (SSN) over email.)

  • Upload valid proof documentation within 31 days of your marriage. Your partner must be legally recognized as a spouse by the state where you live. (See instructions below.).

  • Update your 401(k) Savings Plan beneficiaries.
  • Update your personal information, if necessary, in MyPay (UKG).

  • Consider updating your will/making sure your estate planning documents are in order to protect your spouse and your assets. Get assistance from MetLife Legal Plans if you’re a member.

Divorced spouses are not eligible to be covered under Huber plan benefits. If you get divorced, you must update your dependent’s status to ex-spouse. (See instructions below.)

  • Notify your manager of your return to work and discuss flexible working arrangements, if available.

  • Contact your local HR representative for information about the Company’s bereavement and personal leave policies. employees may take up to three days of paid bereavement leave after the loss of a family member.
  • The EAP can also provide free legal and financial consultation. Legal support includes estate and probate law, real estate, survivor and child benefits, assistance with documents, etc. Financial services include estate planning, budgeting, overcoming debt, bankruptcy, and investments.
  • In addition, the EAP can offer assistance with planning a memorial service, finding child care or elder care, and more.
  • If you’re a MetLife Legal Plans member, reach out for legal support related to estate and probate law, real estate, survivor and child benefits, assistance with documents, etc.

  • Contact the Social Security Administration to end benefits received by the deceased (if disabled or retired) and to determine if benefits are available to the surviving spouse and dependent children.

When you experience a qualified life status event, such as gaining or losing other benefits coverage, you have 31 days to make benefit changes related to your event. Keep in mind:

  • Children up to age 26 can remain on your medical, dental, vision, life and AD&D insurance, regardless of tax dependency, residency, financial support, student or marital status. Coverage will terminate at the end of the month in which your child turns 26.
  • Your adult child will be eligible to continue medical coverage by enrolling in COBRA and paying the full premium amount or you may purchase coverage within 60 days through the healthcare exchange. Learn more about finding coverage through the healthcare exchange
  • Your adult child may lose coverage under the Group Legal Plan at age 19 if they are a dependent of a retiree, at age 24 if they are unmarried and enrolled in an accredited school or college as full-time student(s) or at age 26 if are primarily dependent upon you for support then coverage will continue until age 26.
  • To add/keep a disabled, adult dependent (age 26+) on medical coverage, Aetna must be contacted at 1-866-276-1820 to obtain the forms necessary to verify the disability status (HR Rep can also supply these forms).

  • If you sustain an injury in the workplace, let your immediate supervisor and your local HR representative know immediately. In the event of an emergency, contact 911.
  • Contact your local HR to see if you are eligible for FMLA. If approved, FMLA will run concurrently with Workers’ Compensation benefits.
  • If you are out of work on Workers’ Compensation and do not think you will be able to return to work within a 26-week period, submit a claim for Long-Term Disability (LTD) directly with Lincoln Financial Group during week 16 of your absence. Learn more about the company’s disability benefits.
  • Your basic and supplemental health and welfare coverage (i.e., medical, dental, vision, life, AD&D, and LTD) will continue provided you were eligible and enrolled in these benefits at the time of leave and continue to pay your portion of the premium on a timely basis (monthly direct billing invoices will be mailed to your home address).*
  • Consider electing COBRA coverage, if necessary. After 26 weeks or the end of your approved leave period, if you remain unable to return to work and perform all essential functions of your job (with or without reasonable accommodations), all health and welfare benefits will be terminated at the end of the month.** You have the option to elect COBRA within 60 days, to continue coverage for medical, dental, and vision, if applicable. If you elect COBRA coverage, you will be required to pay the applicable COBRA premiums.
  • You may convert your group life insurance coverage within 31 days to an individual insurance policy with Lincoln Financial Group without taking a physical examination. (NOTE: Your termination of employment will not affect your Workers’ Compensation benefits.) Call 1-844-228-2420 to request the conversion.
  • Access free, confidential counseling through the Employee Assistance Program (EAP).

This is not a qualified life status event. You are not able to change your benefits coverage due to this event.

*You must be actively at work in order for any change in salary and subsequent benefit level to become effective. Therefore, the coverage level for the basic and supplemental benefits (i.e., life, AD&D, and LTD) will remain unchanged over the leave period. If eligible for a pay differential (your HR rep will let you know), your benefits will continue to be deducted from this differential.

**Your termination of employment will not affect the Workers’ Compensation benefits allowed to you by statute in your applicable state.

Retirement*

This is a qualified life status event. Upon your retirement, you may be eligible for retiree benefits through Huber. Log in to COMPASS (or visit COMPASS through single sign-on access when inside the Huber Network if you have already registered) or call 1-844-347-9035 to enroll. You may also be eligible for Medicare and/or Social Security benefits. Learn more on the Social Security website. And, you may have the option of enrolling in other coverage, such as COBRA coverage or your spouse’s benefits. Ask your spouse’s employer for their enrollment process and deadline.

  • QCMSO: Provide your local HR Representative with the Qualified Medical Support Change Orders (QMSCO) and National Medical Support Notices (NMSN).
  • QDRO: Call the Voya Financial QDRO department at 1-904-791-2888. Or, you may send a fax to 1-904-791-2328. You also have other mailing options.
  • Keep in mind: Divorced spouses are not eligible to be covered under Huber plan benefits. If you get divorced, you must update your dependent’s status to ex-spouse and actively submit a Divorce Life Status Event and remove your ex-spouse from all Huber coverage.
  • Remember to review and update beneficiaries for all your benefits, including life insurance and 401(k).

When you are sick or injured, the most important thing is knowing what your time off options are and that your family will be financially cared for.

  • Please see your local HR Manager about how Huber can support you.
  • Talk to your local HR about your options for taking time off due to your own serious medical condition. Ideally, contact HR at least one month before you will need time off. If your time off is unforeseen, please reach out as soon as possible. You may be eligible for job-protected time off and disability insurance coverage while you are unable to work due to your medical condition.
  • Take advantage of Bright Horizons’s Family Care Resources. Backup Care for adults can help provide a caregiver while you are out of work – whether you need help with transportation, household chores while you are unable to move around, or someone to help make sure you are cared for during the day. Learn more.
  • Take advantage of your critical illness benefits with Voya and file a claim for reimbursement, if applicable. If you have questions about your policy, call 877-236-7564.
  • Access counseling services for you and your family members through the Employee Assistance Program (EAP).

Caring for a sick family member can place a lot of stress on you and your family. Take advantage of the resources available to support you during this time.

  • Talk to your local HR about your need to take time off to care for a seriously ill family member. You may be eligible for up to 12 weeks of FMLA unpaid leave. 
  • Take advantage of Bright Horizonss Family Care Resources. Back-up Care for children and adults provides relief for employees trying to juggle work and family commitments. If you are caring for an ailing adult family member, you can also use the Senior Care Planning benefit to help create a personalized care plan as well as help finding caregivers and negotiating rates.   • Contact the Employee Assistance Program (EAP) if you heed help finding movers or other service providers to assist with your move. If you’re buying a home, the EAP can help you find a realtor, lawyer, or other needed professionals.
  • Contact Bright Horizons if you need help finding child care, pet sitters, or housekeepers in your new neighborhood.
  • If enrolled in a Huber medical plan, visit the Aetna website to find health care providers in your new location.
  • Update your corporate credit card billing address, if necessary.
  • Update your state tax withholding information, if necessary.

  • Consider adding pet insurance — you can enroll at any time of year! My Pet ProtectionSM, provided through Nationwide®, offers coverage to help protect against the financial impact of veterinary care.
  • Contact Bright Horizons if you need help finding pet sitters or housekeepers to help you manage your new pet responsibilities.
  • Contact Bright Horizons if you need Back-Up Pet Care. Through the Bright Horizons Pet Care program, you can use a Back-Up Care day to receive a voucher to reserve care for all of your pet care needs (overnight care, dog walking, day care, etc.). Log in to your Bright Horizons Back-Up Care account, select Pet Care > Choose Services > Receive credit to use. Then get the pet care you need.
