To support and reward you for taking actions that encourage good health, Huber provides several wellness programs as part of our Healthy Huber Initiative.

 

Annual Wellness Credits

Beginning in the 2025 plan year, if you are enrolled in a Huber medical plan, you (and your covered spouse) each have an opportunity to earn an Annual Wellness Credit, which lowers your bi-weekly medical contributions from your paycheck. You will now need to complete two important steps to earn the Wellness Credit (for your 2025 medical contributions):

  1. You and your spouse* will need to get an annual physical between January 1, 2023 and August 31, 2024 and
  2. Complete the tobacco-free attestation during the 2025 Annual Enrollment period in the fall of 2024.

Adding the annual physical helps us support and encourage you and your family in taking care of your well-being, getting free preventive care and staying connected to your primary care doctor during the year.

Annual physicals completed in 2023 qualify! After completing both actions, you can expect to see your respective Wellness Credit when you enroll for benefits (during the 2025 Annual Enrollment period in the fall of 2024).

If you are newly eligible for benefits or are a new hire** in 2024, you will receive the Wellness Credit automatically.

Please note

To receive credit for your annual physical exam, your physician (i.e., provider of service) must submit the claim to Aetna as a preventive physical exam, which has specific billing codes for this service. This service can only be a preventive physical health exam, meaning that you are healthy and not sick or experiencing any health symptoms. If your annual physical exam is not coded using these specific billing codes, you will not be recognized as completing the annual physical exam requirement and will not receive the annual Wellness Credit. If the coding is not reflected accurately, you will need to contact your physician to have this corrected and re-sent to Aetna.

*Your covered children are not required to get an annual physical for you/your spouse to receive the credit.

**If you are hired between January and March, you will receive the 2024 Wellness Credit. If you are hired between April and December, you will receive the 2024 and 2025 Wellness Credit. You must complete your physical each year after getting hired to continue receiving the Wellness Credit.

 

Aetna Well-being Rewards Program

If enrolled in a Huber medical plan, you (and your covered spouse) have an opportunity to be rewarded by each earning up to $50 gift cards for completing wellness activities that enhance your overall health and well-being. 

When you complete certain wellness activities such as those shown below, you will receive a gift card reward through Aetna.

Keep in mind: A minimum of $10 in rewards is required in order to redeem gift cards. Covered employees employees and spouses can each earn up to $50 each in gift cards for the year. Be sure to redeem your Aetna Well-being gift card by the end of the calendar year because the gift cards do not roll over to the next year!

To redeem your Aetna Wellness Reward Gift card:

  1. Log into your Aetna account
  2. Under the “Health & Wellness" tab, select “Go to Health & Wellness"
  3. Under Wellness Tools, within the “Discover a Healthier You" box, select “Explore Resources"
  4. Select “Rewards", then “Redeem My Rewards"​

The instructions are the same for the covered employees and covered spouses. The covered spouse would use their Aetna log-in (or register for Aetna first if they are not already registered) to follow and complete the same steps. If your spouse has issues with registration, please take screenshots of the error and forward to huberbenefits@huber.com.

Voya Voluntary Wellness Benefit

If you’re enrolled in Critical Illness, Accident, or Hospital Indemnity Insurance through Voya, you can also earn ADDITIONAL rewards by completing health screening tests! 

  • Covered employees and their spouses can each earn up to $50 in cash rewards annually.
  • Covered children can each earn up to $25 in cash rewards (maximum of $100 for all covered children in calendar year). 

How to Claim Your Voya Voluntary Wellness Rewards

  1. Visit hubervoluntarybenefits.com.
  2. Select "File a Claim".
  3. Scroll down to view the “Remember to file your annual Wellness Benefit Claim" and select “Get Started".
  4. Complete the questions regarding the health screening test, electronically sign and submit your Wellness Benefit claim.
  5. Receive a benefit payment for each covered individual for whom an eligible claim was filed.

Pelago Substance Abuse Management
